Write an essay (250 word min. / 500 word max.) about your mother and why she is deserving of a smile makeover. Be creative!

Submit a color photo of your mother along with her contact information (Name, address, phone, and email).

All entries must be submitted by May 16, 2008. Finalists must be able to come to the office the week of May 19, 2008 for an exam and consultation. Finalists will be contacted by our office to schedule the consultation. The winner will announced and contacted by phone and email after May 23, 2008 and runners up will recieve a free gift.
